To help with organization and to simplify the process of submitting an open records request, we have provided this form and ask that you please provide as much information as possible.

Per O.C.G.A.§50-18-70, our agency is allowed up to three (3) business days from the date of receipt of the request to provide a response; however, if records are not available within that timeframe, you will be notified.
